We welcome new patients to register with us. If you wish to join the practice please bring your medical card (and those of your immediate family who also wish to register) completed and signed to the surgery. You will be asked to complete a health questionnaire, dispensing form and child health surveillance form (for children under 5 years old) if applicable. We would like you to make an appointment with one of our nurses for a health check when you register.  We can only accept patients who live within our practice area.

You will be registered with one of the Partners in the practice. This is an administrative exercise as any patient registered with us is free to see whichever doctor they choose. If you feel strongly about being registered with a specific partner please contact the practice manager.
